Let’s talk about your concerns and questions. I’m here to generally outline how age-appropriate cognitive-communication skills might happen in practical ways for your family during your child’s stages of development. This is also where I will address preliminary questions and guide you to understand what to expect from the therapy process. 

2. Schedule an evaluation

An evaluation is an overview of your child’s current strengths and areas of need. We believe in conducting dynamic assessments that usually consist of a parent interview, clinical observation, review of any samples submitted for the assessment, and a standardized assessment best suitable to evaluate your child’s current skills and get an understanding of norm references for their age group. 

3. Complete case history forms prior to evaluation date

Once an evaluation appointment is scheduled, we will send you intake forms and a patient case history form to fill out prior to your evaluation date. This will be used during the evaluation to determine need for services along with the results from the assessment. Please be sure to bring any additional scans or testing results from other facilities if you would like to share them. 

4. Review results and receive an individualized treatment plan

Most often, results will be shared in person at the evaluation appointment. However, should the need arise where additional samples or assessments are required, results will be shared via phone call by the following day. Here is where we review the results based on norm referenced scores and guidelines. We will then form an individualized treatment plan to best meet your expectations and goals for the speech therapy process with the resources available at Jars of Play Speech Therapy.
